understanding trams

I've had a double mastecetomy. One side prophylactic and the other side cut down to the muscle with a skin graph taken from my thigh. The plastic surgeon is planning on doing a free tram and eventually finishing with implants. I would like to know the difference between a DIEP tram and a free tram. The more I read about it the more confusing it seems to get. Is the DIEP tram and the free tram the same procedure with little variance or are they totally different?

The DIEP does not use any muscle at all.  I had DIEP reconstruction in 2004 and am very happy with the results.  The number of doctors who are experienced in DIEP, though, is limited and may require travel to find someone who really has the training and experience necessary for this "gold standard" of reconstruction.  I went to Mt Pleasant, SC near Charleston for my surgery with Dr Craigie and was very impressed with his skills, his personality, the hospital and staff.

Hi, You need to consult your surgeon and get a proper clinical evaluation done. Did the doctor tell you this was scar tissue? This could even be an abdominal incisional hernia. Only a clinical examination would be able to tell what the cause of your symptoms is and what corrective measures can be taken for that. Do let us know what your doctor tells you. Hope this helps. Failed Tram Procedure

This is going to be a very short response ... Make an appointment with another Plastic Surgeon and have all your records ... this includes the OP procedures and any copies you can obtain from your present Dr's office. I can certainly sympathize with this predicament and perhaps it was your turn to be hit by the "bad luck dart" but that statement is totally inappropriate to my way of thinking. Sounds like an excuse to me as well as the "being too healthy" !! I will say that the numbness in your arm and the scar tissue in the axillary region are pretty much usual after breast surgery and these will most likely be life-long with maybe some improvement in the numbness. Were you not referred for some Physical Therapy following the surgery ?? That's always a good idea when it comes to the degree of flexibility and even now I wouldn't be too late to get an opinion about some prescribed exercises .... I would wait on that until you get some answers from another Plastic Surgeon.  Kindest regards ... don't give up just yet; time does heal any number of problems.

Calcification and MRI

Hi. Just have some clarifications.  By breast x-ray, did you mean mammography?  How is your wife doing now? Calcifications found on mammography need to be further investigated.  The area of clustered microcalcifications should undergo biopsy under ultrasonographic guidance or mammographic guidance.  Magnetic resonance imaging (MRI) can also be used to help localize these microcalcifications and thus aid in the biopsy procedure. There really is no clear guideline on the use of MRI or PET scan after primary treatment of breast cancer.  Patient follow-up with the oncologist is recommended every 3-6 months for the first 3 years of diagnosis.  Mammography is recommended annually or semi-annually.

Tram Flap

Hi there. I believe that the best person that can give you the pros and cons of tram flap will be your plastic surgeon.  Make sure you understand all of these before you undergo the operation.  If in the end you are not amenable to the risks of tram flap operation (longer rehabilitation, some pain, decrease in abdominal wall muscle strength) then you may consider other forms of breast reconstruction (like the use of implants, with skin or nipple sparing operation).  I suggest you discuss this in depth with your doctor. Is my child Autistic?

1. He doesn't speak but babbles a lot now 2. He has communication problems and doesn't really tell us what he wants. He takes our hand sometimes but that's about it. 3. He spins toys, so sometimes when he plays he will spin that toy or spin wheels on a car. This isn't an obsession though. 4. He sometimes eats things off the floor but he is a fussy eater 6. He still doesn't respond to his name most of the time - sometimes it feels like he hasn't heard us. 7. When outside, if set free he will run, smiling at his freedom 8. He sometimes flaps his hands when he wants something, wants to be picked up or is excited. 9. He can become pre-occupied and in concentration especially with the television 10. He used to have sleep problems often waking at night but is much improved now But on the flip side: 1. He looks everyone in the eye and can fix eye contact 2. He shares his joy with us and is always smiling and displaying affection. He laughs a lot and can get very excited when he us having fun. 3. He plays normally with his toys and isn't really obsessive about any one thing, although he can watch TV for long periods 4. He understands when we are cross, or if we are playing, or if we are hiding from him etc. He knows our mood by looking at our face. 5. He loves cuddles and loves being close to us and his grandparents 6. He can take turns in play e.g he can play ball, giving it to us and wait for us give it to him 7. He is very aware of danger and is careful 8. He is not aggressive and doesn't seem to get frustrated 7. He doesn't point to things generally but points to story books and stimuli pictures we show him 8. He doesn't injure himself, bang his head or anything along those lines 9. He isn't compulsive or ritualistic - he doesn't line up his toys or expect certain rituals to be observed 10. He plays hide and seek and will chase us if we run from him 11. If we ask him for a kiss he kisses us, if we ask him to not do something he stops, if we get cross with him he knows he has done wrong and gets upset 12. He is aware of the outside world, loves to watch traffic, cars and lorries 13. He has creative play, he drags his bikes, sits on his trucks and loves playing in play ground swings and slides

Double mastectomy with tram flap

Hi Donna, I have no experience regarding your situation, but it's important that you consult your Surgeon about your symptoms. I know that many women who had this type of breast reconstruction, develop a bulge following Tram flap reconstruction due to weakness in the supportive structures of the abdominal wall. As far as I know,unfortunately this will not go away on its own,but  there is a surgical procedure that can be performed to repair this bulge. You really need to get a clinical evaluation by your surgeon,so he/she can decide what course of management is needed. Best wishes...
