And no you're not going nuts, a lot of Doctors don't really understand when people have things wrong with them and want answers. Especially back problems, there's a stigma attached to it. You're not asking for narcotics so your obviously not a drug seeker, although, be careful with the Tramadol, it some ways it's worse.
And just because they think that CT results don't correlate, doesn't mean that there aren't issues. Sounds like you've been having some pretty serious pain. The Dr's probably not getting back to because he doesn't' know what to say.
Hopefully your Chiropractor can get you through this, I had back surgery about 12 years ago, and had been doing pretty good. 2 years ago,my back went out on me, I could hardly walk, x-rays look horrible. He got me through it, I okay, for now.
If these Doctors don't give you the attention you feel you deserve find another one.
